Violations: A summary of the violations found during the inspection as listed below.
Comments: Complaint: Roaches under soup cup and on floor by salad bar. Also workers kids
(2, about 3 & 4 years old) touching food - cookies and putting them back.
Observation: Did not observe any roaches under the soup cups or on the floor
around the salad bar at the time of inspection. Also did not observe any
roaches in the kitchen area at the time of inspection. Did not observe any
kids touching food and placing back on the buffet at the time of inspection.

Violations: A summary of the violations found during the inspection as listed below.
3717-1-04.3(B) Fixed equipment installation - spacing or sealing.
Fixed equipment did not appear to be properly sealed to the wall in several locations:
1. food preparation sink in kitchen - cracked caulk seal.
2. women’s restroom sink - cracked caulk seal.
3. men’s restroom sink - cracked caulk seal.
Equipment that is fixed shall be installed so that it is sealed to adjoining walls if the equipment is exposed to spillage or seepage. Replace the cracked caulk to provide intact and moisture-resistant seals to the walls.
3717-1-04.4(A)(1) Equipment - good repair and proper adjustment.
Equipment and/or components are not maintained in good working order. The food preparation sink was observed to be improperly attached to the wall and was not level. The backsplash was far enough away from the wall to make the caulk seal difficult to maintain in an intact condition.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Re-attach the sink to the wall so that it is level and close enough to the wall to allow an intact seal.
3717-1-04.5(A)(1) Cleanliness of equipment food-contact surfaces and utensils.
Critical, Corrected during inspection, Equipment food-contact surfaces or utensils are dirty. Observed an accumulation of food residues on the magnetic knife holder.
To reduce the risk of cross-contamination of clean equipment and foods, equipment food-contact surfaces and utensils shall be clean to the sight and touch. Clean this holder.
3717-1-06.1(I) Light bulbs - protective shielding.
Observed light bulbs in the front section of the kitchen that were not provided with proper shields or covers.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed. Provide shields for these light bulbs.
3717-1-06.2(I)(2) Lighting - intensity (20)
The light intensity in an areas used for hand washing or equipment or utensil storage was less than twenty foot candles. Observed one of the fluorescent light fixtures to be “out” with hanging wires.
The light intensity shall be at least twenty foot candles (two hundred fifteen lux): at a surface where food is provided for customer self-service such as buffets or salad bars or where fresh produce or packaged foods are sold or offered for human consumption; inside equipment such as reach-in and under-counter refrigerators and at a distance of thirty inches (seventy-five centimeters) above the floor in toilet rooms and in areas used for hand washing, warewashing, equipment storage, or utensil storage. Repair or replace this light fixture to provide at least 20 foot candles of illumination in this area.
