Health District your source for birth, death certificates


Public Health Week

The Clark County Combined Health Distirct will be providing daily columns this week in conjuction with National Public Health Week.

Do you need a passport? Does your child need to get his driving permit? Has a family member passed away?

These are just a few reasons why you might need to purchase a birth or death certificate. The place to do that is the Clark County Combined Health District, 529 E. Home Road, Springfield.

Since Jan. 3, 2011, anyone born in the State of Ohio can visit the health district and obtain a certified copy of their birth certificate. The cost for each certificate is $22, payable with cash or check only. Certificates may also be obtained through the mail or by using a credit or debit card via the Internet at www.vitalchek.com. Additional fees apply for mail or Internet orders.

At this time, we house death record information only for those deaths that occur in Springfield and Clark County. The cost for these certificates is also $22 per copy, and they may be obtained in the same manner as birth certificates.

The State of Ohio collects fees from each birth or death certificate. Out of the $22 fee, $13.50 is sent to the state. The state distributes this amount to the following agencies: The Ohio Health Department, $9; Clark County Combined Health District, $8.50; the Ohio Children’s Trust Fund, $3; and the Ohio Families Violence Prevention Fund, $1.50.

Also, if you are interested in genealogy, the Health District has birth and death information from 1908 to the present date. We will be glad to provide genealogy information for you using one of the following methods:

  • Walk-in requests will be taken at the Health District's main office, 529 E. Home Road, Springfield.
  • Email requests may be obtained by visiting vitalstatistics@ccchd.com.
  • Fax your request to 937-342-5503.
  • Mail your request to the Health District. Applications are available at our website.

In the near future, we look forward to being able to access death information for all those who have died in Ohio.

Issuing certificates and providing genealogical information is just part of what vital statistics is all about. The Health District registers each birth and death that occurs in our community; works closely with funeral directors regarding death certificates, cremations and burials; assists families with registering home births; provides information for paternity affidavits; provides birth and death statistics to other agencies; and remains dedicated to maintaining and preserving Clark County birth and death information for future generations.
